Approve Agreement to Reimburse Parent for Transportation of Child by Private Vehicle for the Remainder of the 2005/2006 School Year.
EXECUTIVE SUMMARY
The IEP team has recommended the change of placement for the child. The program is located at a different school from the original contract.

Contract amount: $3,226.08
Effective Dates: January 3, 2006 through May 25, 2006
Contract Overview: Please see attached memorandum from Ms. Pam Smith, Supervisor of Related Services with the Transportation Department.

FINANCIAL IMPACT (Budgeted: Yes)
Funds have been allocated in the Transportation Department’s budget for reimbursement of Mr. Vincent Cirrincione for transportation of this student to Colson Elementary (9780;101;7800.391.610.6680). The reimbursement will be based on two round trips (52 miles) at the rate of $.33 per mile each day this student is in attendance for the period of January 3, 2006 through May 25, 2006 ($3,226.08 – estimate based on perfect attendance).
