EXECUTIVE SUMMARY

This Partnership Agreement is in effect September 27, 2006, through September 21, 2007. The STEPS to a Healthier Hillsborough is a grant initiative designed to address the issues of asthma, diabetes, obesity, nutrition, physical activity, and tobacco with under-served populations. Submitted through the Hillsborough County Health Department, it is highly collaborative and provides the school system with the opportunity to support intervention activities in fifty six schools in high poverty areas (Attachment D). Asthma & Allergy Foundation of America will provide education to school staff, recognizing the signs and symptoms of an asthma episode increasing the likelihood of the student in question receiving the appropriate treatment as soon as possible in the school clinic.

ANNUAL DISTRICT GOAL(S) AND CRITICAL SUCCESS FACTOR(S)

Goal 5 – The District will eliminate the Florida Comprehensive Assessment Test (FCAT) achievement gap by a minimum of 10 percentage points as reported by No Child Left Behind.

FINANCIAL IMPACT (Budgeted: Yes)

$3,000 - The source of these funds is from the STEPS to a Healthier Hillsborough Grant.

Evaluations on the subcontractors are done annually. The evaluation for Asthma & Allergy Foundation of America will be completed by October 2007. Asthma & Allergy Foundation of America will provide the list of schools for which they provided education, number of students they reached, dates the classes were held and lesson evaluation completed by the classroom teacher.
