Approve Change Order No. 5 in the Amount of $3,214.97 Extra for
Chiles Elementary School, Classrooms for Kids Package 5

EXECUTIVE SUMMARY

On September 7, 2004, the Board appointed Atelier AEC, Inc. and Christiano Construction Company, Inc. to provide design and construction management services for classroom additions at Brooker, Temple Terrace, and Yates Elementary Schools, and Burns Middle School. On March 14, 2005, additions at Tampa Palms and Chiles Elementary Schools were added to the package. The scope of work provided for the following additions: 18 classrooms at Brooker, 8 classrooms at Chiles, 12 classrooms at Tampa Palms, 14 classrooms at Temple Terrace, 12 classrooms at Yates, and 12 classrooms plus group toilets at Burns. These additions are necessary to meet class size reduction requirements.

The Board approved the Guaranteed Maximum Price (GMP) for the entire package on July 12, 2005. After Board approval of the GMP, the scope of work at Chiles Elementary School was changed to 16 classrooms. When the size of the Chiles addition was increased to 16 classrooms, Chiles was removed from the scope covered by the GMP for the package. The Board subsequently approved a separate GMP for the 16 classroom addition at Chiles on July 12, 2005.

This change order provides for the final reconciliation of the Direct Purchase Program for the addition at Chiles.

This item is included in the District Capital Outlay: Public Education Capital Outlay (PECO) Construction ($776,922) and Classrooms for Kids ($1,827,428).
