SUBJECT / RECOMMENDATIONS

Approve Change Order No. 2 in the Credit Amount of $37,651.02 for
Fuel Station Upgrades at Various Sites

EXECUTIVE SUMMARY

Long & Associates Architects/Engineers, Inc. was selected from the approved Continuing Services Consultants list to provide design services. On April 21, 2009, the Board awarded the contract for construction to UST Environmental Services, Inc. The scope of work provides for the upgrade of existing underground fuel storage tanks and associated equipment at Buchanan and Turkey Creek Middle Schools, Chamberlain High School, and Maintenance East.

This change order provides a credit for the return of unused contingency funds.
