Approve Change Order No. 3 in the Amount of $2,255.42 Extra and Acceptance of Project for
Fuel Station Upgrades at Various Sites

EXECUTIVE SUMMARY

Long & Associates Architects/Engineers, Inc. was selected from the approved Continuing Services Consultants list to provide design services. The scope of work provided for the upgrade of existing underground fuel storage tanks and associated equipment at Buchanan and Turkey Creek Middle Schools, Chamberlain High School, and Maintenance East. On April 21, 2009, the Board awarded the contract for construction to UST Environmental Services, Inc.

This change order provides for final reconciliation of the Direct Purchase Program.

The architect advises that the referenced project is finally complete in accordance with the plans and specifications and other contract documents and recommends Board acceptance.
