EXECUTIVE SUMMARY | |
This multi-year contract renewal is in effect July 1, 2011 through June 30, 2013. Categorical funding is provided annually through the State General Revenue dollars. These contributions are used to provide funding for 20 school registered nurse positions, fringes, travel, supplies, and training (yearly amount is $485,802). | |

The total contribution of $971,604 ($485,802 for each 12-month period) is from the General Appropriations Act, Human Services Budget (State of Florida General Revenue Dollars). |

EVALUATION | |
The annual report and plan are submitted to the State of Florida, Department of Health. Data is collected from monthly reports submitted by school registered nurses. | |
