Approve the Agreement with Florida Heiken Children's Vision Program
EXECUTIVE SUMMARY
The Florida Heiken Children's Vision Program (FHCVP), a Division of the Miami Lighthouse for the Blind and Visually Impaired, recently received a contract from the Florida Department of Health to provide financially disadvantaged children enrolled in Florida's public schools comprehensive eye exams and glasses, when prescribed.  There is no cost to the students or their families.  The student criteria for an eye exam and glasses, if prescribed, is listed below:

  • Student must be enrolled in the school lunch program
  • Student must have failed the school vision screening twice (does not have to be on a separate day)
  • Student must have no insurance
  • Student must have a parental consent form signed by the parent/guardian

The Title I schools to be served will be Giunta and Shields Middle Schools.

Funds received from a contract through the Florida Department of Health
EVALUATION
Dr. Bryan Wolynski, Optometric Consultant to the Miami-Dade Program, and his staff will conduct a statewide assessment after two years of serving students in Florida.
