Approve the Renewal of Early Childhood Transition Plan with the Bay Area Early Steps Program of the University of South Florida (Part C Provider) and Hillsborough County Public Schools (Part B Provider) 2019/2022 (Academic Support and Federal Programs) | |
EXECUTIVE SUMMARY | |
This Interagency Transition Plan, effective since 2001, ensures a collaborative transition as mandated by law including the involvement of family members in planning/completing the transition process from Part C to Part B. The transition process addresses educating families about transition and ensures that needed evaluations are conducted for children receiving Part C services by their third birthday. |

EVALUATION | |
The above named lead agencies meet quarterly to monitor the timely transitions and conduct an annual review at the end of each fiscal year. Performance is measured through State Performance Plan Indicator 12. | |
