Accept the Highest Ranked Proposal Submitted by Lakeside Occupational Medical Centers P.A. - RFP No. 4648-TW Professional Services To Provide Physical Examinations, OTETA Drug Testing, and Random/Post Accident Drug/Alcohol Testing (Business Division)

EXECUTIVE SUMMARY

On June 14, 2005, the Purchasing Department in conjunction with the Safety and Risk Management Department issued a request for proposal (RFP) for Professional Services to Provide Physical Examinations, OTETA Drug Testing, and Random/Post Accident Drug/Alcohol Testing. Firms were asked to provide their knowledge/experience, work plan and references. Proposals were due on July 14, 2005. A committee of District personnel was formed to review the proposal responses. Three proposals were received within the RFP timeframe and were evaluated by the committee on July 19-20, 2005. Based upon the RFP specifications and committee evaluations, it is recommended that Lakeside Occupational Medical Centers P.A. be accepted. The effective date for this award is September 30, 2005. The initial term of the contract shall be for a five-year period. This contract may be renewed for two additional one-year periods provided all terms and conditions remain unchanged. The base price shall be firm for the first twenty-four months from the date of award. Thereafter, prices may be adjusted in accordance with the Consumer Price Index at the end of every twelve-month period. The percentage of change shall apply to the face amount of the contract, which shall include all labor and materials. Pricing shall not increase more than 5% in any one year. To ensure fair and open competition, the District utilizes an online vendor registration system to notify potential bidders of opportunities. Through this system, 120 vendors were notified of the solicitation, three proposals were received, and 117 did not respond.

As stated in OPPAGA report: “Efficient purchasing and warehousing requires that management processes be in place to ensure that supplies, equipment and services vital to the school’s education mission are purchased from a competitive source, in the right quantity, delivered timely to the correct location, and stored in a secure location. These criteria should be met for each purchase without sacrificing quality.

FINANCIAL IMPACT (Budgeted: Yes)

These items are, or will be, included in the budget for the Safety and Risk Management Department in an amount of $230,000.00.
