Approve Agreement for Continuation of Reimbursement to Group Home for Transportation of Child by Private Vehicle for the Remainder of the 2005/2006 School Year
EXECUTIVE SUMMARY
The Individual Education Plan team has recommended the District reimburse the group home to transport the student to and from Willis Peters. The contract includes:

Effective dates: October 3, 2005 through May 25, 2006

FINANCIAL IMPACT (Budgeted: Yes)
Funds have been allocated in the Transportation Department’s budges for reimbursement of Florida Mentor for transportation of this student to Willis Peters (9780;101;7800.391.610.6680). The reimbursement will be based on two round trips (14 miles) at the rate of $.33 per mile per day this student is in attendance for the period of October 3, 2005 through May 25, 2006 ($1,339.80 – estimate based on perfect attendance).
