Approve Agreement to Reimburse Facility for Transportation of Child by Private Vehicle for the 2006 Extended School Year.
EXECUTIVE SUMMARY
The IEP team has recommended the District reimburse the facility to transport her child to and from Willis Peters. The contract includes:

FINANCIAL IMPACT (Budgeted: No)
Funds have been allocated in the Transportation Department’s budget for reimbursement of Florida Mentor for transportation of the student to Willis Peters (9780;101;7800.391.610.6680). The reimbursement will be based on two round trips (28 miles) at the rate of $.33 per mile each day this student is in attendance for the period of June 5, 2006 through June 29, 2006 ($175.56 - estimate based on perfect attendance). Total: $175.56
