Employee Benefits Open Enrollment 2007 and the Humana Wellness Program’s Online Health Assessment.

EXECUTIVE SUMMARY

Open Enrollment for 2007 Employee Benefits begins Wednesday, October 4, 2006, at 8 a.m., and ends Wednesday, October 11, 2006, at 12 midnight. Eligible employees may choose to participate in one of eight medical plan options (HMO Staff, HMO, EPO, POS, CoverageFirst, PPO, HDHP or Opt Out) and/or one of three dental plans. Employees may also enroll in vision, disability, life, cancer, hospital income protection, GAP coverage plans, and medical and/or dependent care flexible spending accounts (FSAs) during this open enrollment period.

Employees who do not want to change any of their current benefit elections do not have to take any action (exception: FSA participation). All current enrollments in FSAs terminate December 31, 2006, even though participation continues through March 15, 2007. If any employee wants an FSA for 2007, he/she must make a new FSA election online (or call the Aon helpline) during the Open Enrollment period. Beginning October 4, 2006, employees may enroll in their benefits by visiting the enrollment website at www.sdhc.selfservicenow.com, attending one of the open enrollment meetings (Armwood, Chamberlain, and Bloomingdale High Schools), or calling the Aon Helpline at 1-866-522-9133.

Also, on October 4, 2006, Employee Benefits is kicking off its Humana Wellness Program “Get Healthy," featuring a personal health analysis via Gordian Health Solutions. All health enrolled employees are being asked to participate in the Humana online health assessment by entering www.myhumana.com, registering, and clicking on “Have you taken Humana’s Health Assessment?” anytime between October 4, 2006 and November 11, 2006. Participating employees will receive immediate online health status reports and will have access to Gordian coaches to help them meet their "Get Healthy" goals. During the year Employee Benefits will receive summary reports, not identifiable by employee, to help assess the wellness of the district as a whole. This will be an annual assessment to help us identify the health and wellness areas needing attention and to assess our progress over time.
