APPROVAL OF CHANGE ORDER NO. 4 - $7,656.00 NET CREDIT

EXECUTIVE SUMMARY

This project was planned by the architectural firm of Robbins, Bell & Kreher, Inc. On February 6, 2001, the Board appointed the construction management firm of Beck, Ltd. On December 11, 2001, the Board approved an interlocal agreement with the State of Florida authorizing the School District to oversee the design and construction of a health clinic adjacent to the new County Social Services Center. To date, the State has committed funding in the amount of $2,428,931. On January 13, 2004, the Board approved the Guaranteed Maximum Price of $2,233,008.

This change order provides a credit for deletion of exterior canopy for the Women, Infants and Children (WIC) area. It also provides gutters and downspouts for the high canopy between the Clinic and the Social Services Building.

Goal 6 - All schools in the district will be assigned a state grade of A, B, or C.
