Approve Change Order No. 5 in the Amount of $9,517.04 Extra for
Frost Elementary School, New Facility
EXECUTIVE SUMMARY
This project was planned by the architectural firm of Cooper Johnson Smith Architects. On November 18, 2003, the Board appointed the construction management firm of Batson-Cook Company. This new facility was a reuse of the Bevis Elementary School design.
The Board previously accepted this project. However, an error in the reconciliation of the Owner Controlled Insurance Program (OCIP) for the project was subsequently discovered and validated. This change order provides an adjustment to the OCIP reconciliation, to correct the error.

Provide adequate school facilities for the accomplishment of all District Goals and Critical Success Factors.
