Approve Change Order No. 1 in the Credit Amount of $29,330.42, Change Order No. 2 in the Net Credit Amount of $10,673.89, and Acceptance of Project for Flooring Maintenance, Classrooms for Kids Addition at
Cypress Creek Elementary School

EXECUTIVE SUMMARY

On November 9, 2006, the Board appointed Long & Associates Architects/Engineers, Inc. and Mathews Construction to provide design and construction management services for Classrooms for Kids Additions at various schools, including Cypress Creek Elementary School. On August 7, 2007 the Board amended the agreements and approved a separate Guaranteed Maximum Price (GMP) to add flooring maintenance at Cypress Creek Elementary School to the project scope. The scope of work for the flooring project included installation of vinyl composition tile (VCT) in existing carpeted and VCT areas throughout Cypress Creek Elementary School.

Change Order No. 1 to the flooring maintenance project provides a credit for the direct purchase of materials by the District, in accordance with the Direct Purchase Program. Change Order No. 2 provides for final reconciliation of the Direct Purchase Program, and credits for project savings and return of unused contingency funds.

The architect advises that the referenced project is finally complete in accordance with the plans and specifications and other contract documents and recommends Board acceptance.
