Annual Employee Benefits Open Enrollment for 2009

The Annual Employee Benefits Open Enrollment for 2009 begins Wednesday, October 1, 2008, and ends Friday, October 10, 2008. Employees may attend any of three Open Enrollment meetings, each of which will include a health fair. These meetings will be housed in the media centers of Riverview High School, Jefferson High School, and Tampa Bay Tech on October 2, October 4, and October 8, respectively. At these meetings, employees can meet with the various benefits vendors, have their elections entered online, take the Humana Health Risk Assessment online, participate in the Humana health fair, and even get their annual flu shots.

This year each employee will have three ways to elect benefits:

• Call the Aon Call Center to have elections entered.
• Go to the website at www.sdhc.selfservicenow.com to enter elections online.
• Attend one of the three Open Enrollment meetings with Health Fairs to get all benefits questions answered and have elections entered.

Details concerning all Open Enrollment information will be distributed to employees prior to September 29, 2008. A supply of materials will be produced in Spanish, and Spanish-speaking Univers enrollers will be available at the three Open Enrollment meetings with Health Fairs.

No additional cost to the district; enrollment costs are included in the products's premium rates.

Health Insurance is renewed annually.
