Approve Change Order No. 3 in the Amount of $36.19 Extra and Acceptance of Project for Roof Replacement at
Van Buren Middle School

EXECUTIVE SUMMARY

Reliable Group Architects was selected from the approved Continuing Services Consultants list to provide design services. On April 21, 2009, the Board awarded the contract for construction to Quality Roofing of Florida, Inc. The scope of work provided for roof, edge metal, and roof drain replacement on selected buildings.

This change order provides for final reconciliation of the Direct Purchase Program.

The architect advises that the referenced project is finally complete in accordance with the plans and specifications and other contract documents and recommends Board acceptance.
