Approve the Addendum to the Memorandum of Agreement Between The School Board of Hillsborough County, Florida (HCPS), Florida Department of Health, Hillsborough County (DOH), Tampa Family Health Centers, Inc. (TFHC), Suncoast Community Health Centers, Inc., and MORE HEALTH, Inc. School-Based Sealant and Fluoride Program
EXECUTIVE SUMMARY
At the August 12, 2014, School Board meeting a Memorandum of Agreement was approved to provide dental sealant, fluoride varnish prevention and sealant education to head-start, second and third grade students attending a selected group of schools, beginning September 12, 2014. This addendum is to offer services to all elementary schools with a high enrollment of students that qualify for free or reduced school lunches (Title I elementary schools), beginning October 1, 2014.

• Demonstrate "best practices" in all aspects of safety including students, employees, visitors, facilities, transportation, and health
FINANCIAL IMPACT (Budgeted:  No)
No financial impact to the district.
EVALUATION
Annual reports will be submitted to DOH with information regarding dental screening and follow-up.
