| Approve Amendment #002 to Contract #HB331 to Full Service Schools with the State of Florida, Department of Health (DOH) (Student Services Division) | |
| EXECUTIVE SUMMARY | |
| Approve amendment #002 to the 2014/2015 Full Services Schools contract #HB331 that was approved at the School Board meeting on June 24, 2014. Its purpose is to expand school-based health services and increase access to health care in high poverty areas. The Full Service School funds provide direct health related services to schools in our district. This is an annual renewal of flow-through funds from the Hillsborough County Health Department to the school district and has been in effect for over 20 years. The amendment was based on the Department of Health, Hillsborough's request to the Department of Health, Florida for extension of scoliosis screening date and collection of emergency information forms.
For your convenience due to the size of this contract, we have attached a separate page showing all changes. It is the practice of the DOH to sign all documents after School Board approval. |
