Approve Acceptance of Project for Modular School-Based Health Center Clinic at Hunter's Green Elementary School
EXECUTIVE SUMMARY
The District was previously awarded a grant for replacement of an existing School-Based Health Center Clinic housed in an obsolete portable on the campus of Hunter's Green Elementary School. The $500,000 grant was awarded under the Affordable Care Act (ACA) Grant for School-Based Health Centers Capital Program, administered by the U.S. Department of Health and Human Services Health Resources and Services Administration. The grant provided for replacement of the portable with a 1,728 square foot modular building housing exam rooms, a dispensary, restrooms, offices, and a conference/training area. District staff selected Long & Associates Architects/Engineers, Inc. from the approved Continuing Services Consultants list to provide design services. On January 21, 2014, the board awarded a contract for construction of the clinic to Halfacre Construction Company. The design professional has certified that the work is complete in accordance with the plans and specifications and other contract documents and recommends Board acceptance.
