Provide Explanation of Form 1095 as Required under the Affordable Care Act (ACA) (Human Resources Division)
EXECUTIVE SUMMARY
Form 1095 is a new tax form required under the Affordable Care Act (ACA). Beginning with the 2015 tax year, this form will be required for annual tax filings. The form documents the months of the year employees and/or their dependents were offered or enrolled in medical coverage. Form 1095 is required as proof that an individual was adequately insured each year as required under the ACA. If an individual is not covered, tax penalties may result.

Employees will be receiving their Form 1095 at the end of January. They will be delivered to school sites and will be mailed to employees who are retired or no longer with the district. In addition, all active employees will have access to this form through the district’s Employee Self Service (ESS) site.
